Missed Appointment Fees:

A $60 No-Show Fee will be charged for each missed 30 minute appointment.

A $120 No-Show Fee will be charged for each missed 60 minute appointment.

Three No-Shows will result in dismissal from the practice.

 

Medication Refill Requests:

Our providers will endeavor to ensure at each appointment that patients have adequate prescriptions until their next office visit.  Should the need arise for a prescription refill or transfer outside of an office visit, please contact our office directly regarding your request.  Please note that our office does not receive communication from pharmacies regarding refill requests. We will review your chart to evaluate your request and, if appropriate, your provider will assess a $25 service fee and send a refill to the pharmacy.

Lab Fees:

Patients who have their blood drawn at our office agree to pay Vibrant America Laboratory for lab processing. Vibrant America is a self-pay laboratory and does not bill insurance. Patients may request an outside lab order if they prefer to use their insurance for lab processing.  Patients may be assessed a $50 fee for working with an outside lab. Additionally, we are unable to assist with any billing issues, including insurance coding.
